Creative Independent Living Ltd is a CQC registered Supported Living provider authorised to provide accommodation, social, community and personal care and support to adults and young people.

 

The range of work varies from personal care to social/community care, night sits, sleep in-s and re-ablement. Our client groups varies as we support 18-64 age group, 65+, adults with learning disabilities and mental health problems, people with sensory impairment and physical disabilities


Our ethos is based on ensuring partnership and collaborative working, therefore joint assessment and referral systems are being utilised where appropriate, to avoid duplication. We take an individual approach to each person that we work with and their wider family members, as we know that there isn’t a single solution to improving outcomes for individuals.


Our holistic care planning process assesses the needs of the whole person (i.e. physical, psychological, social, emotional and spiritual). This will require our staff to spend time on gaining an understanding of the individual’s life history, personality, mental and physical health, relationships, attitudes and aspirations. The planning or provision of any service is approached from the individuals and family carer’s perspective.


Our care planning process is intrinsic to developing the personal care plan which assists in maintaining the quality of life, empower individual to exercise their rights, encourage them to make a positive contribution, the process is free from discrimination and harassment.

With almost 10 years of proven experience, we offer a package of services that is flexible and adaptable to meet individual needs.

We recognise the importance of service users ability to have control over certain aspects of their care including, staff gender and preparation of meals etc. We proactively collate cultural, personal and social preferences and match these to the most appropriate staff member.

 

Involving and engaging the individual and family/representative when developing the support plan ensures person centred planning takes place. In this way the individual’s needs/ family carer’s needs are identified, strengths, preferences and aspirations are identified. They are actively empowered to exercise their rights and have control over their choices and level of engagement.

We have information in accessible formats on the services we provide and a library of information around services that are available in the wider community e.g. leisure, community events, community safety events, wellbeing sessions such as exercise and healthy eating. These opportunities are proactively discussed with clients and their relatives /representatives before developing a personalised support plan. This gives individuals/family carers choice and control in the care they need and when and how they want the service delivered.

 

The personalised support plan would focus on what the client is able to do and what assistance they need, the plan will support with enabling independence, as well as support with wider health and wellbeing benefits such as mental and physical wellbeing through social networks, consultation, engagement, involvement.

 

 

We support and encourage all clients to make a positive contribution, both to their community and to the development of our services and company. All clients are supported to engage with and make full use of their community facilities and to maintain close links with family and friends. The support we provide may be practical such as providing transport and accompanying clients during visits, or it may be helping this find information about and make contracts with suitable local groups, events and venues.

 

We also encourage all clients to contribute to our service improvement by sharing their views and giving feedback and in more direct ways such as participating in interview and selection processes, taking part in open meetings and client discussion groups or participating in training sessions with new starters.

The personalised support plan focuses on what the client is able to do and what assistance they need.

The plan will support reading and promoting independence, as well as providing support with wider health and wellbeing benefits such as mental and physical wellbeing through social networks, consultation, engagement and involvement.

 

Where necessary, we will work flexibly with the whole family to provide realistic and manageable solutions. An example of this is where an individual is keen to access the local leisure centre for swimming sessions. He would usually attend these with his parents but as part of promoting his independence he is keen to attend on his own. The care worker would find out times of sessions and bus routes, escort the individual to the swimming sessions and back home. Eventually over time we would work in partnership to develop the individual’s independence and confidence to be able to travel on their own and access the facilities.
