Transition, 16-18 services

The transition to adulthood can be especially challenging for young people with learning disabilities and complex needs, and for their families.

 

This can lead to unsuitable placements, social isolation, well-being concerns, and the use of residential care or other costly out-of-area placements.

 

We can support young people aged 14+ to have well-planned and person-centred transitions to adulthood. This includes young people with learning disabilities and/or autism and other complex needs. We can support their families through this transition too.

 

Our aim is for young people with learning disabilities and complex needs to achieve successful transitions to adulthood. Specific outcomes are that young people:

 

  • are at the centre of planning and well-prepared for the change process
  • make informed choices about their transitions
  • acquire independent living skills
  • live with or near their families
  • are connected to their local communities
  • are engaged in good-quality education, employment or training
  • have good physical and mental health during and after the transition.

Outcome-focused transition plans will be developed with the young person and their families. We view co-production of service design as an essential part of delivering high-quality support.

 

Experienced Support Workers will support young people, both one-to-one and in small groups, helping develop independence and life skills in line with their unique needs and to complement their Education, Health and Care (EHC) plans.

 

This will include independent travel skills, health and wellbeing, personal safety, employment and training options, cooking and shopping, money management, friendships, social skills and connecting with and being part of their community.

 

Support Workers will support young people to set goals and regularly monitor their progress. Support can be time-limited or ongoing throughout the transition period.

 

We will place young people at the centre of decision making and seek to bridge the gaps that can exist between agencies by working in partnership.
